List of Owners, Directors, and Partners:

Information about the directors, partners, or owner, like as names, contact details, and addresses.

List of Workers:

A list of employees along with their details, such as name, gender, date of birth, pay details, date of joining, etc.

Cancelled check or bank statement:

To enable the electronic transfer of ESIC contributions, a copy of the bank statement or a cancelled check is required.

Provide Evidence of the Business:

records attesting to the business’s address, such as utility bills, property tax receipts, or lease agreements.

passport-sized pictures of each employee and the employer.
